I use Avidemux mainly to convert YouTube flv to mp4, and so far it has been working perfectly without any loss of quality. I use the "Copy" video/audio feature. However, the mp4s always have the filename added as a caption (or a title) while the original flvs did not. All I do is I manually remove the captions from each file.

How I can disable that?

Thank you.

With vlc open the mp4 when video is playing press Pause
Now from vlc menu: Tools > Media Information
in title field erase the contents and save.
Or you have to use a program like mp3tag
Could be there is a solution in Avidemux to change that behavior?

To save yourself some time, you might try using movgrab to get the file you want from youtube rather than getting a flv and converting it.  As an example so you can see that youtube always creates multiple filetypes:

$ movgrab -T http://www.youtube.com/watch?v=eDK-EJPM_e4
