Why is there no MP4 option in my save?!?!?!?

What settings did you choose for:

• Video Output
• Audio Output
• Output Format

Because you probably (your screenshot hides the relevant area of the Avidemux window) have chosen the MKV muxer for output in Avidemux. Choose the MP4 one and you will get .mp4 (and lose .mkv) in the filtered file dialog.
